Making a Claim
If you need to make a claim under your MPPI policy, then please contact our Claims Team on:

We will then send you a claim form, which you must fill in and return to us as soon as possible together with any further evidence needed. Alternatively, you can download an MPPI Claim Form below:
Please note: Please do not return incomplete claim forms to us as this will significantly delay your claim.
You will also need to provide the following information:
- If you are unemployed - independent evidence that you remain registered with a benefits office as being unemployed and available for work unless exempt from registration. We will also require proof that you are still actively looking for work.
- If you are disabled - independent evidence that you are still disabled and that you remain under the regular care and attendance of a doctor or consultant. We will require proof that you are not receiving any form of payment or reward (other than your normal or statutory sick pay).
